how do you avoid using well-word words or cliches in poems/writing in general? I love to write but sometimes I find myself slipping into such phrases and I can't really find an appropriate substitute for them sometimes

It helps to write out a long list of clichés in a notebook, like “her eyes were as deep blue as the ocean," you know, certain phrases or words that are overused. And then write an alternative phrase or word next to the cliché one, something you can substitute for the cliché.

So for example, instead of writing “her eyes were as deep blue as the ocean," you could write next to it, “her eyes were as deep blue as the water at the bottom of a well you could drown in forever."

Then, after you’ve finished the list of clichés and their substitutes, continue on writing as normal, and whenever you catch yourself writing a cliché word or phrase, check your list and replace it with a substitute. c:
